Parents,

Each year, we need a completed Medical Permission Form for your son.

Included in the form are areas where you can indicate emergency contact information, list any allergies we need to be aware of, and a list of over-the-counter medications that your son will be allowed to receive.

We have also included the concussion and sudden cardiac arrest forms that are required for participation in PE in 4th, 5th, and 6th grades.

Information you'll need to gather to complete the form:

• Letter from physician stating they can participate in P.E. for 4th, 5th, and 6th
• Immunization Record Scan (new students and Senior Kindergarten)

With more than 30 years of experience helping children and families, Heidi offers practical, compassionate guidance for understanding what true readiness looks like—not just for school, but for life.

During this session, parents will gain helpful insight into how children develop the foundations for classroom success, how to set developmentally appropriate goals, and how to support each child’s unique path toward confidence and independence. This event is designed to equip parents with simple, practical strategies they can use at home as they help their children thrive in learning, relationships, and everyday life.
