Required Medical Forms - Due Friday, June 14

Parents,

Each year, we need a completed Medical Permission Form for your son.

Included in the form are areas where you can indicate emergency contact information, list any allergies we need to be aware of, and a list of over-the-counter medications that your son will be allowed to receive.

We have also included the concussion and sudden cardiac arrest forms that are required for intramural and Crusader sports.

If there are no changes to the medical information, there is an option to indicate this on the form.

Information you'll need to gather to complete the form:

• Letter from physician stating they can participate in athletics for 4th, 5th, and 6th
• Immunization Record Scan (new students and Senior Kindergarten)
