Required Medical Forms for Back-to-School
Parents,
Each year, we need a completed Medical Permission Form for your son.
Included in the form are areas where you can indicate emergency contact information, list any allergies we need to be aware of, and a list of over-the-counter medications that your son will be allowed to receive.
We have also included the concussion and sudden cardiac arrest forms that are required for participation in PE in 4th, 5th, and 6th grades.
If there are no changes to the medical information, there is an option to indicate this on the form. You will still need to scroll down to the submit button.
Information you'll need to gather to complete the form:
- Letter from physician stating they can participate in P.E. for 4th, 5th, and 6th
- Immunization Record Scan (new students and Senior Kindergarten)

‍Tenn EFS Application Portal Opens Tomorrow | As announced in April, Presbyterian Day School will participate in the Tennessee Department of Education’s Education Freedom Scholarship (EFS) program (vouchers) for the 2025–26 academic year. The state’s application portal for families will open May 15. To learn more about eligibility and how to apply, visit the official program website.
A Reminder of Important Details- The EFS program is managed by the Tennessee Department of Education. PDS has no input on who does and does not receive these funds.
- The proposed voucher amount for 2025–2026 is $7,295 per student.
- Families with rising Senior Kindergarten through 6th Grade boys will be eligible to apply. For the 2025–2026 school year, 10,000 vouchers will be available for eligible students who meet certain household income limits, and 10,000 will be available for eligible students, regardless of household income.
- Scholarships will be awarded on a first-come, first-served basis based on when the Department receives a completed application.
- The EFS application portal is expected to open on May 15. Here is a checklist of how to apply.

ArtWalk - Art by Every 1st-6th Boy on Display at PDS
The Library hallway has been converted into an art gallery featuring original art from every boy in the 1st-6th grades!
Parents are invited to pop into the school this week to celebrate the work of all our artists.
Art will be on display through Friday, May 16. |

Summer Library Hours
Our library will be open this summer on Tuesdays (12–3pm) and Thursdays (9:30am–12:30pm) June 3-July 31 for book check out and AR testing.
The 2nd-6th grade students will be able to take AR tests at home using the login information included in their summer work folders.

Important Update: Student Account Payments for 2025–2026
Beginning with the 2025–2026 school year, all payments on student accounts will be processed through Blackbaud. Parents created a Blackbaud login when signing their contracts for the upcoming school year.
Please note that the first payment on all 2025–2026 payment plans is due June 1. Statements were sent earlier this week from Blackbaud so please check your email.
For any outstanding balances from the 2024–2025 school year, payments should still be made via Bill & Pay. All 2024–2025 balances must be paid no later than June 1. Your Bill and Pay statement will be emailed 5/20/2025.

Open Gym Dates for Rising 4th-6th Grades
Hey Rising 4th – 6th grade Parents,
I have mentioned to the boys that I will be supervising open gym time for boys who want to play pick-up games with their peers this summer. I will not be coaching or picking teams. Boys who wish to improve their basketball skills must get a feel for the game, which comes from playing. They are more than welcome to bring friends who do not attend PDS. Drop your son off at the 2PC Recreation entrance.
